Myocardial Infarction 


Myocardial Infarction (MI):


Definition:

Damage to heart muscle due to reduced blood flow, resulting in:
1. Necrosis of heart tissue
2. Impaired cardiac function

Types:

1. ST-Elevation Myocardial Infarction (STEMI)
2. Non-ST-Elevation Myocardial Infarction (NSTEMI)
3. Unstable Angina (UA)

Causes:

1. Atherosclerosis
2. Thrombosis
3. Embolism
4. Vasospasm
5. Trauma

Risk Factors:

1. Hypertension
2. Hyperlipidemia
3. Diabetes
4. Smoking
5. Family history
6. Obesity

Symptoms:

1. Chest pain (radiating to arm, neck,jaw)
2. Shortness of breath
3. Fatigue
4. Lightheadedness
5. Nausea and vomiting

         •Sweating

         •Nausea , Vomiting

        elevation/depression                   
       •Troponin levels (elevated)
       •Creatine kinase (CK-MB)
       •Myoglobin
       •Imaging studies (echocardiogram,  cardiac MRI)
 
Dx :
       • Myocardial Infarction MI 

     

            Rx:

                  •Oxygen inhalation through nasal mask
 
                  •Pain management morphine IV ×STAT  

                  •Percutaneous intervention if  the patient presented within 90 mints 

                  •Thrombolytic therapy with in 12 hours (Streptokinase , Alteplase)

                   •Tab: Aspirin 81-375 mg P/O× STAT 

               

                   •Tab :Clopidogril 75 mg P/O × STAT

                  •Tab :Atenolol /Bisoprolol 12.5mg ×PO ×OD

               •Tab :Captoril 25 mg P/O×BD

                   •Tab :Atorvastatin 40/80     mg P/O OD

                      

     Instructions:

                     • Lifestyle modification(diet, exercise)
                     • Try to avoid stress,anxiety 
                     • Risk factor management
                    • Regular check-ups
                     • Aspirin therapy(if indicated)
